I'm not sure what the pay type is.All I see is Name,Address,Apt/Suite,City,State,zip and Country,then submit..?

Is it asking for like a payment method? If you email me your name and address I will put it in for you and see if that helps..???

I'm getting that same "pay type" message. It pops up on the screen after you enter your name and address and click "submit".
