I have an enormous medicine cabinet and a lot of it is expired or will expire before I manage to use it up. As long as you are willing to do research on how the drugs' potency changes over time, I don't see why these things should go in the trash - many pharmacies in the US just automatically stamp a one year expiration date on drugs regardless of what they are or when the original bottle they came from actually expires. Even OTC drugs often have very short expiration dates when the drugs don't really change much.

NSAIDs generally don't expire as long as you keep them in a cool and dry place. I still take aspirin that I bought around 2000 and it works, I just take twice as much. As long as it isn't something that will become dangerous, like tetracycline, I keep it until it's gone and buy more BEFORE I need more. Nothing worse (in my mind) than being sick and not having what I need on hand.

(I even prefer some expired drugs sometimes because I don't want such a strong effect.)

It's also nice to have a good pill cutter, empty gel capsules, gauzes, and whatever other non-expiring supplies you might need.
