$5. Send 3 UPCs from 12 packs or 2 UPCs from 12 packs of 16.9 oz bottles of any combination of the following: Pepsi, Diet Pepsi, Caffeine Free Pepsi, Caffeine Free Diet Pepsi, WIld Cherry Pepsi, Mt. Dew, Diet Mountain Dew, Sierra Mist and/or Aquafina. Send a DCRTC (dtd by 3-8-05). The form says you can send the mail in certificate or your NAZ on a 3 x 5 card. Req’d store form Rec’d by 3/18/2005

I just hope they're still good and they take it without a form...It's worth trying though if you normally buy them anyways
