"Rebate form must be returned with valid cash register receipt dated by 4/30/04 with UPC code and purchase price circled for bread product... Offer valid to residents of AK, CA, GA, IN, MN, MO, ND, NE, OH, OR, SD, WA and of legal drinking age... All submissions must be postmarked by 5/31/04... No wine purchase necessary."
