Here is a recipe my kids love to make
yogurt large container (32 oz)
frozen concentrate juice container (11.5 oz or 12 oz)
Dump both ingredients in to a container with lid that can be put in the freezer. mix together well. snap on lid and place in the freezer.
Ready to eat when frozen. Spoon out with a warmed ice cream scoop. Serve in bowl or in cones.
Personal notes : We use 100% juice concentrate Rasberry grape juice is our favorite with Peach white grape juice a close scond.
We use vanilla flavored yogurt. I tend to buy several of the tubs when I see them at the store. Usually, we use them all up before the expiration date. However, we tend to make the frozen yogurt then we haven't been able to eat them all and the expiration date is quickly approaching.

Do you thaw out the juice concentrate before combining with the yogurt does it mix it somehow already frozen? (I think this might be a dumb question. )

Not a dumb question at all
We have used the juice concentrate thawed and still frozen
You just have to stir longer if you use the still frozen concentrate.
We stir until we get a consistent color
