Dew, Diet Mountain Dew, Sierra Mist and/or Aquafina. Send a DCRTC (dtd by 3-8-05). The form says you can send the mail in certificate or your NAZ on a 3 x 5 card. Req’d store form Rec’d by 3/18/2005
